Great solution for older monitors and new GPUs

This cable is was exactly what I needed. Keep in mind this is a directional cable, so it only works going from to DP to HDMI and not vice versa.

FYI - Cable is Directional

If you need the opposite connection, like I did, this cable will not work for you. It will not work if you need to connect the HDMI end to your computer and the Display Port end to your monitor.
