iTunes is only used to buy things from the iTunes Store or Apple store (such as music, ringtones, movies, games, or other applications). Whoever has another Apple product, the video call will be free since it is based on the Apple software. If you need to make a telephone call outside of country you should contact your service provider (Verizon, AT&T, Sprint, etc) or purchase a out of country call card. For example, my grandmother speaks with family from Mexico I go to the liquor store and buy call cards for Mexico which is cheaper than paying out of country calls.

FaceTime is free, wether is video or just audio calls, you don’t have to pay nor have money in your iTunes account; all you both need is internet and both need to be using apple devices (iPod touch, iPhone, iPad, MacBook, Apple Watch, etc). I hope this helps.
