3 possibilities that I've found to happen; your car is too hot, car's too cold, (both with the casette in the car during the more extreme conditions), or it is or it's about to break.

If you are inserting the cassette player upside down, it could be interfering with the tension controllers within the adapter. If the radio does not see tension, it thinks that the tape is broken and will eject it. Try turning the cassette player over. 2. The tension system within our adapter is missing a part and not providing the tension the radio needs. Replace the adapter to see if this corrects your issue. 3. There is an issue with the tension portion of the cassette player itself. If step 1 or 2 doesn't fix the issue, this is the most likely cause.
