Yes it does twist. It is very tight when you first get it. You can lefty-loosy or righty-tighty it. Loose means the chair will tilt backwards easier. If you tighten it all the way it will be next to impossible to tilt backwards. If you make it too loose you could tip back too easily, BUT you have to crank and crank it to get it to be too loose. So cranky away, test it, then crank again until you like it.

Maybe it's very stiff but it is supposed to turn. yours might be already turned 100% in one direction or the other, but it should definitely move in at least one direction (clockwise or counterclockwise). It probably will not be super convenient to do while sitting on the chair. Perhaps get off the chair to do it. Keep at it. It is a very common adjustment in office chairs of any kind.
