I had the same issue. You have to go on otter box website and submit through them directly.

The case was almost 2 years old. When we contacted Otterbox, they asked us to send them photos of the case and then quickly replaced it with a new case -- holster and all.

However, in my experience on small items like this, the Best Buy store you bought it from will likely switch it out for you even though they're not actually obligated to.
