• Yes there are a variety of bolts for wall mounting.

• Yes, 4 lag bolts are included. They require a 1/2" socket or driver for installation. I highly recommend that you pre-drill for the lag bolt to eliminate the chance of splitting the wall stud.

• Yes they do and the template helps. Just measure and make sure you are level

• This is the one thing that I purchased separately because I wanted a better quality lag anyway. I believe there were lag screws in the package, there were tons of other screws for just about every option.
