Does this TV support someone hearing impaired listening through bluetooth headphones while the rest listen through a surround system via eARC or must the second output source be the TV speakers?

When this function is active, you can set the volume of the Bluetooth headphones higher than the volume of the TV speaker. First, make sure your Bluetooth headphones are paired. Press HOME on your QN90A remote. Then go to Settings > General > Accessibility > Multi-output Audio. Find the device you'd like to pair. ^Gina

If you are asking about Multi-Output Audio (output audio to the TV and a bluetooth audio device at the same time), than yes, that feature is an accessibility feature that should be available.
